Collapse of an existing maxillary fixed partial denture resulted from periodontal disease and caries
in a 50-year-old patient. Clinical and radiologic examination revealed advanced periodontal disease with
considerable bone loss and multiple root caries with separation from the crown portion of the teeth. It
was decided to provide this patient with an interim maxillary complete denture.
Impressions of the maxillary and mandibular arches were taken with irreversible hydrocolloid. An
irreversible hydrocolloid impression was made in a disposable stock acrylic resin tray. The borders of the
tray were corrected with modeling compound. The accuracy of reproduction of the border outline and
other essential anatomic structures was examined.
The impressions were poured with dental stone. Occlusal registrations, made by an established
method, were used to mount the working casts on the articulator. The maxillary working cast was
modified by removing the maxillary teeth. An acrylic resin maxillary base was preformed with an opening
to allow insertion of the patient's porcelain teeth. Occlusal pattern resin index (GC) was used to
determine the occlusal registration.
The remaining teeth were extracted and the fixed partial dentures were removed. The porcelain
fixed partial denture in the articulator was inserted in the opening in the acrylic resin base in accordance
with the occlusal pattern resin index. Retention was prepared in the porcelain teeth; the gap between the
teeth and the base was filled with wax to stabilize the teeth; and stock acrylic resin denture molars were
added to complete the arch. The denture was then processed, trimmed, and polished. The interim denture
was inserted and relined with soft tissue-conditioning material.

Discussion
Interim dentures, also referred to as provisional, intermediate, transitional, or treatment dentures,
do not require separate appointments for the extraction of anterior and posterior teeth. All the teeth are
extracted simultaneously and an interim denture is inserted.

The transition period allows for maximum
contouring of the residual ridge before the definitive prosthesis is placed.
Use of the interim denture ensures that there will be no further esthetic compromises of the
patient's dentition. The patient is usually satisfied with the esthetic appearance of the natural teeth or the
fixed prosthesis. Tooth shade, the position of the teeth in the arch, and morphologic contours of the natural
teeth or fixed prosthesis may be duplicated on the interim denture.

The existing interocclusal contacts
should be maintained. Few, if any, changes in tooth position, occlusal contacts, and cuspal inclinations are
found with interim dentures.

The interim denture not only helps patients overcome the psychological
trauma associated with the loss of natural teeth but also provides a biologically more acceptable
transition from the dentulous to the edentulous state.
